Welcome to the Dr. Nick Brown Dental Care family. Most of our general dental appointments take about an hour; however, because we value our patient relationships so much, we like to take a few extra minutes during the first visit to get to know you and discuss any questions or concerns you might have.
During your initial visit, you’ll receive a comprehensive visual exam and professional cleaning. If Dr. Brown makes any diagnoses, he will also give you a clear, honest explanation of your dental condition and treatment options, so you can make an informed decision about your care. It’s always our aim to develop a treatment plan to match your dental health needs and personal goals.

New Patient Forms
To help our team facilitate your first visit, please print* and complete our Patient Registration and Health History forms, and bring them with you to your appointment.
*We also have a kiosk in the waiting room where patients are able to quickly fill out forms at their first visit.
For Your First Visit

Please bring the following items with you to your appointment:
• Dental Insurance Card (if applicable)
• Identification such as Driver's License, Military ID or State ID
• List of medications

That would be ideal! If you are able, please arrive 15-20 minutes early to fill out any remaining patient forms.

This can vary, but you should probably plan on 1 to 1.5 hours for the first visit. After making an appointment to see Dr. Brown, you’ll arrive to find a friendly and caring staff with many personal touches to keep you comfortable. You can enjoy television or bottled water in our lobby. We’ll take a few digital x-rays. In our comfortable patient rooms, you’ll have a seat in our massaging dental chairs and you can listen to music while our hygienist cleans your teeth. Dr. Brown will visit with you for your dental exam.
At Dr. Nick Brown Dental Care, we believe in taking a comprehensive approach to your dental care. This includes a personalized treatment plan, oral cancer screening, periodontal exam, home care technique assessment and more. Then we’ll schedule your next visit with us. Yes! All patients will receive ONE late cancellation or no show “freebie”. After that, if patients fail to give us at LEAST 24 hours notice, they will be charged a $50 late notice fee. We strive to value every patient’s time and deliver the best care possible, so we ask that you please value our time as well.
